Trenton NJ, the long-feared warnings about a sharp rise in healthcare costs have materialized as the 2026 Open Enrollment period begins. New Jersey residents relying on the Affordable Care Act (ACA) marketplace, employer-based coverage, and Medicare are all facing significant premium hikes, leading public officials to fear a massive surge in the number of uninsured residents next year.

The most dramatic increases will hit the nearly half-million New Jerseyans who purchase insurance through the state’s marketplace, Get Covered NJ.

Obamacare “Subsidy Cliff”: Costs Soar 175%

The most alarming jump is tied directly to the political battle on Capitol Hill. The enhanced federal subsidies, which have shielded millions from the true cost of ACA plans, are slated to expire.

  • The Sticker Shock: Without a last-minute deal in Congress, the average premium increase for the 466,000 subsidized residents in New Jersey is estimated to be more than $2,780 annually, or 175%.
  • The Political Factor: Hope for a deal plummeted after a Senate agreement to end the government shutdown on Sunday failed to include an extension of the enhanced ACA tax credits, only guaranteeing a future vote.

Governor Phil Murphy voiced serious concern, stating, “Many actions taken by the federal government are having and will continue to have a negative impact on our residents’ ability to afford health care.” Experts fear this massive price spike will force many to forgo coverage entirely in 2026.

The Underlying Causes of Rising Healthcare Costs

Beyond the political subsidy fight, a mix of economic and health factors are driving costs up across the board:

  1. Inflation & Care Utilization: Rising costs of medical care, labor, and supplies due to general inflation.
  2. Drug Prices: The escalating cost and enormous demand for high-cost specialty medications, particularly GLP-1 weight loss medications.
  3. Aging Population: An older population requiring more complex and frequent medical services.
  4. Insurer Profit Strategies: Companies are tightening their plan designs to contain costs and maximize profit.

Employer & Medicare Users Not Immune

Even those with employer-based coverage or Medicare are facing unprecedented increases:

  • Employer-Based Plans: Premiums are expected to rise by 6% to 7%—the highest increase in years, according to consulting firm Mercer. Furthermore, 59% of employers plan to offset these costs by raising employee deductibles and out-of-pocket maximums.
  • Medicare Premium Spike: Medicare Part B premiums (covering doctor visits) are projected to rise about 12% to $206.50 (up from $185). This increase is about double that of the prior year, significantly outpacing the projected Social Security COLA increase of just 2.8%.

For seniors, the pressure is intensified as insurance companies managing Medicare Advantage are shifting clients toward more restrictive HMO plans and away from PPO options to better contain costs. As one advisor noted, “The pendulum needs to swing the other way, because it’s gone too far against the patients.”

    They still have a very low net premium.
    Notice the article doesn’t tell you what the average premium is.
    The regular ACA subsidies remain intact, the Covid enhanced subsidies are ending.
